A teenager was killed and three others were injured in a two-car collision in Oldham County early Wednesday, officials said.

Oldham County Police and other agencies responded about 8:20 a.m. to the 6200 block of West Highway 42, the department said in a statement, and found a sedan and a pick-up truck "which were involved in the collision" and "were off the roadway."

On Friday, the police department said that 42-year-old Theresa Devine was driving a 2011 Dodge Ram eastbound in the westbound lane. She crashed with a 2016 Mazda 3 driving westbound in the westbound lane.

A passenger in the Mazda 3, 16-year-old Lily Fairfield, died at the scene, police said. The driver of that car, 19-year-old Zoe Fairfield, and the two people who were in the Dodge Ram — Devine and 40-year-old Joshua Waits — were hospitalized.

An investigation is ongoing, and anyone with information is asked to contact the Oldham County Police Department at 502-222-1300.
